My combo

Muwahaha i am so evil

I had jace memory adept on the battlefield and did his last ability on me and my friend.  In my hand among other stuff got a card with graft and tibalt.  I played tibalt then the creature with graft, got him to 4 loyalty counters and then did his second ability doing 24 damage with 5 mana.

Graft only applies to +1/+1 counters. Loyalty counters on planeswalkers are not the same. You cannot graft counters onto tibalt

and graft affects the creature coming into play after the graft creature is in play

Sure, if you could somehow get him to enter as a creature.  When a permanent becomes a creature, it's not entering the battlefield, so it won't trigger things like Graft.  Now, if you somehow got it to happen ({Mycosynth Lattice} + {March of the Machines}, for instance), you could graft the counters, but they're still just +1/+1 counters, so they'll only buff him while he's a creature.

No, because graft is an etb ability:)

Why not then? He stays a creature until end of turn, summon your graft in the second main phase.

graft states that the creature enters the battlefield wirh x counters. then when another creature enters the battlefied you can move a counter. there is no way to play the creature with graft after the first creature and still get a counter on it.

There are cards that do what Giant Fan does, IIRC. I know there's {Power Conduit}.
