Wondering what can/should go and what to add.

I'm considering adding {Snapcaster}.

I wonder too, if I need protection for  {Nekusar, the Mindrazer}?

Yes.
And, yes. Nekusar WILL be the main target of your table, and you WILL draw a lot of hate for playing him, but he is a great commander.

Some cards you should add:
{Basilisk Collar} - "Oh you're drawing cards? Have some damage! Oh, Nekusar has Lifelink..."
{Grafted Exoskeleton} - "Draw 10 cards and you die."
{Howling Mine}
{Jace Beleren}
{Reforge the Soul}
{Wheel of Fortune}

etc.

I'm wondering if I need to find a reliable way to protect him...the only way to win is to have him sit on the board.

The build reminds me of Niv-Mizzet, but without the combos...which is easy to ensure victory..

Arzair

In response to the "do I need to protect my commander" the answer usually depends on what kind of build you have. This build looks like he is your main wincon and so I say ABSOLUTELY!

After playing with my nekusar deck my playgroup now knows that he is a MAJOR threat and is usually the target of many attempted counter spells or 1 for 1's... I even got a wrath played when he was the only creature on the field because of "the potential threat"

Yeah he draws a TON of hate. I'd wager he draws more hate in commander than Mind Sculptor does. Which is saying something.

{Kami of the Crescent Moon}

I guess I will have to actually play test in my group before I can make judgements.

Basically, if you want to play Nekusar, kill everyone fast, otherwise they'll single you out and make you the first one to go.

Caininite

 {Library of Leng} is good to have in a draw/discard deck.

He's not nearly as powerful as Niv, so I'm hoping my group will risk him on the board to draw extra cards.

I might add that and  {Spellbook}.

Arzair

 {Bloodchief Ascension} is amazing in my nek deck, goes "on" very fast and really makes those wheels fun.

Arzair

 {Psychosis Crawler} dies whenever you wheel doesn't it? Because at one point you will have 0 cards in hand? This is the only reason I don't play him, someone correct me please if I am wrong.

This is a rules question, but the answer is no. State-based actions are not checked until a player receives priority, which is after {Wheel of Fortune} is done resolving. {Psychosis Crawler} would live.
