Junk Tokens 75 cards, 15 sideboard 2 {Gavony Township} 2 {Temple Garden} 2 {Overgrown Tomb} 2 {Godless Shrine} 4 {Forest} 2 {Evolving Wilds} 2 {Swamp} 2 {Plains} 2 {Vault of the Archangel} 20 lands 2 {Trostani, Selesnya's Voice} 2 {Armada Wurm} 4 {Avacyn's Pilgrim} 1 {Voice of Resurgence} 9 creatures 4 {Intangible Virtue} 4 {Farseek} 3 {Midnight Haunting} 4 {Lingering Souls} 4 {Advent of the Wurm} 1 {Garruk, Primal Hunter} 3 {Ready // Willing} 1 {Devout Invocation} 2 {Sorin, Lord of Innistrad} 4 {Call of the Conclave} 1 {Collective Blessing} 31 other spells Sideboard 2 {Rootborn Defenses} 4 {Duress} 1 {Pithing Needle} 1 {Sundering Growth} 2 {Loxodon Smiter} 3 {Selesnya Charm} 1 {Collective Blessing} 1 {Emmara Tandris} 15 sideboard cards I built this over the weekend and I would like some tips on how I can make this better and post rotation tips will be help full as well

Post rotation you'll have to drop black and do green white or naya

It's really hard to say. If you do Naya your options open up alot, but that means harder mana fixing.  {Assemble the Legion}  {Molten Birth}  {Hive Stirrings}  and {Young Pyromancer} are all decent options, the only hard thing for tokens is going to be stuff like  {Gaze of Granite} and  {Ratchet Bomb} both being in standard, also not having  {Intangible Virtue} really hurts. Look into some of those cards and see what you like, if you go RW you can put the new Purphoros god in, which will make all your tokens that come in deal 2 damage to the opponent.

You forgot the 1st/2nd biggest token treat; Supreme Verdict lol.

Consider  {Path of Bravery} and the new white god's artifact that is an anthem. Also one of the best things to help counter  {Supreme Verdict} is  {Golgari Charm} with it's regen effect, and it has enchantment destruction for the new set. Junk tokens will probably still work, but it won't be as strong as it once was.

Replace rootborn with {Golgari charm}. More versatility + lower CMC at the expense of landbase problems.

Having four colorless lands makes casting things like Trostani and Advent of the Wurm pretty hard. I would drop the Armada Wurms and Devout Invocation and up your land count by at least two or three. Possibly also drop the Vaults for more color-producing land.
