So certain creatures have abilities that deal damage to a player, correct? Well is it the creature dealing the damage? Or is it damage from nowhere like a sorcery or instant?

It's the creature. Just like if a creature attacks a player the creature is responsible for the damage.

{Zhur-Taa Druid} for example

So "niv-mizzet, the fire mind "is considered to be that way since he deals one damage for everytime that player draws a card? Correct?

To add damage from a sorcery comes from the sorcery. If you have pro-red ({faith's shield} at under 5 life for example) a red spell like {lightning bolt} cant deal damage or successfully target you.

{Faith's Reward}? Do you mean  {Faith's Shield}?
