1. Would {increasing confusion} and {rest in peace} cause an infinite mill?
2. Do cards ever technically go to the graveyard or is it straight to exile?

1. Quick answer, no.
My guess is you are looking for a standard version of the  {Helm of Obedience} mill trick.
The reason that works and increasing confusion wouldn't is because the helm stops when a certain requirement is met, which can never take place. Where as confusion just counts the top amount from your library and then puts them in the gy.

2. It's a replacement effect. So the first card sees it as going to the graveyard, then rip changes it's location.

(I may be slightly incorrect but that's it as far as I understand how it works)

Interestingly, this is the first ruling from rip:

10/1/2012   While Rest in Peace is on the battlefield, abilities that trigger whenever a creature dies won't trigger because cards and tokens never reach a player's graveyard.
