Does equipment like {Trepanation Blade} get to be equipped right away or do the have summoning sickness?

They will. You can still move the weapon itself around as needed, though

How does that work?
Do you just pay the equip cost again?
What about equipment with both attach and equip like {Cranial Plating}?
Does equipment stay on the Battle field after the equipped creature dies?

As long as you have the mana you can exchange it around.

Attach is instant speed as equip is sorcery

Equipment is a separate permanent and needs to be destroyed for it to leave the battlefield.  Attach is an ability and those are instant which can be used anytime even during opponents turn.  You must pay equip cost anytime you want to equip or re equip to another creature and can only be done during your main phases when the stack is empty.
