Not in my experience. But I'm not familiar with the rules WotC endorsed with the release of the Commander decks.

Nope. If you cast Phage from the command zone you will lose, unless {Platinum Angel} is rocking the field next to her. Or your opponent has an {abyssal persecutor} staring at you. Could also try {Lich's Mirror} if you're feeling dangerous.
