So when you add +1/+1 or -1/-1 counters to a creature. And it dies. Do the counters fall off when it enters the graveyard? Or if you were to return them to the battlefield with a creature say {Sun Titan} would the creature come back with any counters it had?

All status (including counters) are cleared as the creature changes zones (play to graveyard)
