Double strike is not stackable. Two instances of double strike are redundant.

That list has fortify but not equip, and poisonous, but not wither or infect.

That is because poisonous is a truggered ability while wither and infect are damage modifiers. Not sure about the fortify thing though as far as i know it is not possible to get multiple instances of fortify or equip onto an object anyways.

So poisonous is better than infect in the case of dealing damage to players because it can be stacked and it also deals normal damage.

{Puresteel Paladin} gives a second instance of equip. They are not redundant.

Because you have stacked to seperate instances of scavenge on one card. By stacking were refering to if it can be present multiple times on a card within the realms of the rules. For instance the game does not allow for deathtouch to be on a card multiple times, it either has it or it doesnt.
