Would milling one teammate out cause them both to lose or do they both have to be milled out?
Both. If one player loses, the team loses.

What is milling?
Forcing an opponent to discard cards from they're library. It can be a win con. As you know, if you have to draw a card from your library, and there are no cards, you lose. So milling would be to get rid of your opponents library. Also called decking. I believe the term "mill" is from a card. Cant remember which though
{Millstone}

Just to be clear, cards aren't exactly "discarded" from the library, so things like {Millstone} won't work with {Liliana's Caress}. Otherwise all good.
Ah yes, discard wrong word to use. There isn't just one word for it is there? "from the top of your/opponents library"
