A friend play this combo in a B/U zombie deck, the match is lose when he play it 😔

Not at first, no, actually that's pretty funny. I just thought of what "trepanation" actually is, and made a connection there

Would double strike activate the ability of the sword twice?

If both hits connect with the player, yes.  But the Trepanation Blade only triggers once, when you declare attackers.
