The only thing this would do is resolve before the opponent has to choose to pay life the first time around.  Then it will re-enter and the trigger starts over.  More or less it would be a waste to cast {cloudshift} since they will only have to respond to one trigger either way.

Sure. By why would they take the first round of damage?

I don't think they would have a choice to take damage is what I'm saying.  If cloudshift exiles the creature then the etb ability is next on the stack would it fizzle or can they take the damage even though the creature is not around to be sacrificed

I know it's a waste of the card I'd just like to have a better understanding of the rules

Tge first ETB trigger would not fizzle.  They could take the dmg for the first instance but why would you since the creature is not there?

So once they decide to take damage the creature is sacrificed and does the 4 damage go on the stack also?

If you read the wording, if the player doesn't take any damage nothing happens.
