If I have out  {Basilica Screecher} and  {Syndicate Enforcer} then I cast a spell, do I pay the extort cost once and they lose 2 life and I gain 2? Or do I have to pay it for each creauture with extort?

For each instance of extort you may pay 1 mana, so, say I have 4 creatures with extort out, i play a spell, I'd have to pay 4 mana  for each creature's extort ability.

The situation above,
If you paid 1 mana, each opponent would lose 1 life and you'd gain 1 life, if you paid 2 mana, each opponent would lose 2 and you'd gain 2

Each instance of extort uses the stack and you decide if you are going to pay the cost when the ability resolves. In the above example with 4 creatures with the extort ability, when you cast a spell {healing salve}, for example, each creature puts its extort ability on the stack. So, the stack looks like {healing salve}, extort, extort, extort, extort. This first means that damage/health gain from extort will resolve before the spell that triggers it. Second, as each resolves you can either pay or not pay, so you can choose to pay 2 mana and let the other two resolve dealing no damage. Players will also receive priority after each instance of extort resolves, though most of the time this won't matter.
