The {Primeval Titan} seems a bit out of place here, plus 6 CMC is pretty expensive for a 6/6 creature with an ETB that isn't that relevant for you, though it does work pretty well with {Hedron Crab} which is a good modern mill card.

I wanted to try and get some ramp into the deck to advance myself a turn or more ahead for the mind grind and to help get nelhalia out when I got to prime Titan. Had a soft spot for him and always enjoyed him.

I'd cut the green ({Primeval Titan} and {Farseek}) in favor of {Glimpse the Unthinkable} for more milling power and {Thoughseize}s to get rid of threats before they hit and buy you the time you need to mill them.

I'm more of a standard player than modern so kinda flimsy when it comes to modern builds.

Creatures and spells that are just decent in Standard don't cut it for modern usually. They need to be quite exceptional since there's so a large pool of good cards to use that aren't available in standard.
