Hi I'm thinking about upgrading my phone but I don't want to lose all of my data from this app what can I do to transfer my info?

Back up to Dropbox and then load from it. That's how I sync my iPhone and iPad.

To the best of my knowledge, there are three ways you can back up data.
1) (easiest method) Dropbox. Very intuitive and simple system, just upload data to it and then download it onto your new device via the cloud storage. Here's a link to getting you started. Dropbox works on Mac and PC also. https://www.dropbox.com/mobile
2) (slightly confusing, I just tested it) back up app data to iCloud. This does require you to use iCloud. Here's a link to start you off on that route: http://support.apple.com/kb/HT4910
3) Sync your iDevice to ITunes, then use a 3rd party software (such as iExplorer, link below) to extract the files, then put them into the correct spot in the new device. I really don't suggest this unless 1 and 2 ended up not working, as opening up your idevice's file system and tampering with it could result in you having to restore the phone.
