What do you use to alter the cards?
What kind of markers/paints?

Anything you feel would work or look good. I personally used "citadel" layer paint. The same paint that my lgs sells for the warhammer figures and what not. It was the easiest thing to find. I do suggest useing a water based latex paint though. It's thinner and will allow you to just start painting where as a thicker say oil based paint you'll have to thin out alot to keep the card as close to playable legal as possible.

Do you actually paint onto the card with brushes?

If you are looking to do foil alters or trying to make foil blanks: Avacyn Restored has the easiest ink to remove off of foils. Use nail polish remover and it comes right off.
