With the "sacrifice when targeted by spells/abilities" on the card, does that trigger each time it's targeted by a spell or only the first spell/ability that targets it?  {Phantasmal Image}

In example, if it's targeted by a law keeper's tap ability, can I unsummon it to my hand or even give it undying? Or does the trigger prevent that by triggering again because of another spell targeting it?

You would have to sac the image before any effect from a card could resolve so using your example with {Gideon's law keeper} if you use { unsummon} or {undying evil} you would target the image making you have to sac it then either of the cards would resolve with no active target. Hopefully I made sense.

Any time a "Phantasm" ({Phantasmal Bear} etc) is targeted by any spell or ability it will set up a sacrifice trigger. You can't {Unsummon} them, {Giant Growth} them, equip them, or {Undying Evil} them. In each case, the illusion will be sacrificed before those spells resolve.
A global, non-targeting ability, like from {Mikaeus, the Unhallowed} would work.

Even if it's more than one at once.

It only triggers once, as the stack resolves the image is no longer there to be targeted.it only triggers once.

It will trigger once for each time it is targeted by a spell or ability.  If your opponent targets it with {Gideon's Lawkeeper}, it will trigger.  Then if you respond with an {Unsummon}, it will trigger again.
