How does {Trepanation blade} work in 2 HG?

When you attack in 2hg, you select which "head" you are attacking. Whichever head you select would be the defending player and the sole target of the mill effect, even though both teammates share blockers.

This also causes spells like {sphere of safety} to be useless in 2hg - you can just keep attacking the opponent you don't have to pay mana to attack!
