Does {cather's crusade} trigger for each human, or just the card when you play {thatchers revolt}? And does {unruly mob} get a counter for each token dying? Or just one since they die at the same time?

You get a counter for each token that enters, and each token that dies. Each token is a seperate entity, its own creature. 3 tokens enter the battlefield, so 3 creatures trigger the effect. Same deal with them all dying.
