It's not infinite mana and infinite tokens, but a combo I ran in my casual deck was {Mana Echoes}, {Krenko, Mob Boss} and {Volcanic Geyser}. You tap Krenko to produce tokens, you produce mana and then you blow them up for the win.

It is infinate mana from what I can see... You tap one to remove a counter, and put a 1/1, then that adds 2 mana, do it again and it adds 3, again and 4, and so on... And it won't run out of counters due to {Caesar's Crusaid}
