So if you suspend an {Illusory Angel}, and it is the first (or only) suspension that resolves that turn, what happens to the angel?
I would assume you cant cast it if you havent cast anything before it.

You would try to cast it, but the rules would prevent you from casting it from exile. So it stays.
Correct me if I'm wrong but things that are exiled once they return to the battlefield they are not "cast" just returned. So the cast trigger should not trigger, yes?

you're thinking of flickering and oblivion ring type cards, completely different mechanics here.

When a card with suspend has the last counter removed from it, the player then must cast it if possible. This is different from an effect like {Cloudshift} that just brings it to and from exile.
Also, it is a static ability, not a trigger. Effects that don't involve casting can still put the angel in play regardless of spells cast: {Quicksilver Amulet}, {Doomed Necromancer} etc.
