i have {Accursed Witch} in play

opponent casts {Flaying Tendrils}

just want to make sure where it goes or what will happen to Accursed Witch before i play fnm later

I believe that {flaying tendrils} creates a replacement effect which prevents the witch from ever "dying" and thus transforming. So she would not turn into the aura and instead become exiled

This.

The Tendrils replacement effect means the Witch never goes to the graveyard which is required for a "dies" trigger.

Yep. To die = to hit the graveyard from play.

For that never happens because of a replacement, she doesn't return.
Likewise, if she's exiled (from the graveyard) with the trigger on the stack, she won't return either.
