Title: Transform

1) Transforming does not cause the permanent to leave the battlefield correct?
2) Transforming does not cause the permanent to untap unless it says so (see demon land)?

Example:
3) A {Bloodline Keeper} that {T}s to make a 5th vampire token cannot then {B} and Transform and {T} again to attack or make another because he is still tapped correct?

Totally true, indeed. Transform is the specific game action of turning a double sided card other-side-up. This doesn't cause it to leave the battlefield (although the flip walkers are exiled and return transformed), nor does it affect a permanent's status. (Tapped, Flipped (From Kamigawa), face up/down, phased in/out).

110.6. (http://imtgapp.com/forum/index.php?action=imtg;area=rule;number=110.6.): A permanent's status is its physical state. There are four status categories, each of which has two possible values: tapped/untapped, flipped/unflipped, face up/face down, and phased in/phased out. Each permanent always has one of these values for each of these categories.
