Yes. You gain all the life lost that way. But not from something like {Guttersnipe}, that would deal damage when the spell is cast, because the damage comes from another source.
To clarify: if you are playing a 4-man game and you cast it for X=5, you have three opponents that each lose 5 life, meaning 15 life was lost that way. Therefore, you gain 15 life.

In a 4 player game if x = 5 your 3 opponents would lose 10 life and you would gain 30 life; debt says they lose life equal to 2 times x.

At three life left they still lose 15 to get to -12 life

Yup. In a four-player free-for-all, each Extort trigger you pay for will net you 3 life, barring effects like {Platinum Emperion}. If one opponent had one of those guys out, you'd only gain 2, since only two life would be lost.
