It's actually five on him. I had something just like this posted earlier with {sage of hours} for a turn-three extra turn

Where does the fifth counter come from?

Targeting him triggers heroic for two counters with scales. Panaply doubles the two, then gets one more from scales for 5 total.

Panoply doesn't double. It adds one.
