Yes. The living weapon effect is a trigger upon ETB that an be Stifled.

They are written as "[Trigger condition], [effect]," and include (and usually begin with) the word "when," "whenever," or "at." See rule 603, "Handling Triggered Abilities."

The ability phase was way before my time. So if you phase the token out anything attached to it goes with it?

Correct, leaving the battlefield don't count either.
I'm confused...why does the attached equipment go with the phased token? Wouldn't batter skull just fall off and sit like a regular equipment? What if you {heroes downfall}ed the token?

702.25f (http://imtgapp.com/forum/index.php?action=imtg;area=rule;number=702.25f): When a permanent phases out, any Auras, Equipment, or Fortifications attached to that permanent phase out at the same time. This alternate way of phasing out is known as phasing out "indirectly." An Aura, Equipment, or Fortification that phased out indirectly won't phase in by itself, but instead phases in along with the permanent it's attached to.
