Title: Through the breach

If I counter a {through the breach} after a card from his hand has been named, would that card go to the graveyard, or return to his hand?

The card is only named upon resolution of the spell. If the spell never resolves, you never put a creature anywhere, and the creature would remain in your hand.

If it was something like  {Tormenting Voice} where you gotta discard a card as part of the cost, even if it gets countered you still have to discard the card.

If a card works like this, it will specifically say "as an additional cost"
