Things you'll need:
Acetone (commonly used in Nail Polish Remover)
Q-Tips/Cotton Swabs/etc
A crap foil (Premium Deck Series cards seem to work fine)
Printable Transparencies/Window Decals

It's pretty simple.  You just use the acetone to wipe the ink off of the card, leaving you with a very shiny, blank foil.  Be careful to not let much of the acetone seep over the sides or on to the back, since it might cause the foil layer itself to start feeling off, or the ink on the back of the card to run.  Then, you print your intended proxy/artwork/whatever on to a Transparency or Window Decal, making sure to resize it to the size of the card.  Let it dry, then stick it on there with whatever adhesive is needed, and there you go!

Personally, I used PDS: Graveborn foils and self-adhering window decals from Staples.  The decals are a little thicker than what you want, so if you actually try to figure it out, you could tell it's a foil proxy inside a sleeve.  I believe I used an Inkjet printer, though, so the ink stays on just fine.  If you decide to use a Transparency with some sort of glue, I'd recommend printing the card so that you can glue it on Ink-side down (ink between the Transparency and the foiling), since that'll make it less likely for the ink to be smudged after it dries.

thats how i do it but if you use spray glue you want to barely put any glue on the card or the ink will do funny things that looks like it exploded on the card

Just depends.  Rtr works fine, mirror in, the actuall foil comes off, a couple old ones won't de foil, and some old ones just come right off.
I found a list once that showed what sets were good for de-inking and what sets to avoid due to them simply blanking and not leaving even foil. Not sure where, but I do know that Scars and Innistrad block are no-nos, (as in, do not use them to de-ink) unless you are trying to obtain blanks.
