If my {murderous redcap} already has 1 (-1/-1) counter on it, does playing {Melira, Sylvok outcast} remove it?
Nope. Melira just stops further counts from being added, but doesn't remove ones already there.
Also backed up by a gatherer ruling - 6/1/2011: Melira doesn't remove any poison counters you already have or any -1/-1 counters already on creatures you control.
