Title: needs clarification

To the best of my knowledge +1/+1 counters and -1/-1 counters offset each other. So if I control {Mikaeus the Unhallowed} and {Murderous Redcap} and a sac outlet is that infinite?

Yep. They cancel each other out. Here's the ruling.

121.3. If a permanent has both a +1/+1 counter and a -1/-1 counter on it, N +1/+1 and N -1/-1 counters are removed from it as a state-based action, where N is the smaller of the number of +1/+1 and -1/-1 counters on it. See rule 704.

Do both persist and undying trigger though? I would think you select which goes on the stack first?

Correct: you only get one since redcap can only be returned once per grave visit, but it is still infinite if you always choose undying to resolve first.
