So when you add +1/+1 or -1/-1 counters to a creature. And it dies. Do the counters fall off when it enters the graveyard? Or if you were to return them to the battlefield with a creature say {Sun Titan} would the creature come back with any counters it had?

The existence of {Skullbriar, the Walking Grave} can be used as proof if you need to easily show someone and don't want to flip through the ruling numbers ;)
