Title: Sac Undying

If I sac a creature with undying without a +1/1 counter on them do they come back or are they gone because of the sacrifice?

Yes they come back. Undying triggers don't care how they were put into the GY. Just that they're there and didnt have any +1/+1 counters on them.

Actually it says dies. So if an Undying card is discarded or milled, it doesn't come back with a +1/+1 counter.

If it comes from the battlefield to the graveyard, in any way, the trigger activates

Yes I should've specified that it must be from the battlefield into the graveyard. Nice catch.
