Draw your entire deck, and get a bunch of mana you can dump into a huge {fireball} or something
{krenko mob boss} + {intruder alarm} = infinite goblins
Edit: {galvanoth} + {brain storm} = free spell
Edit: {niv-mizzet, the firemind} +....there's a card were whenever you deal damage to a player you draw a card. Anyway, it equals infinite damage

This is especially effective in multiplayer as {Guttersnipe} damages all opponents so you get significant card draw
I always found that {Niv-Mizzett, the firemind} has a best friend named {consecrated sphinx}. Soo good in MP

No. It's only considered cast if the card specifically says the copy is cast. See {isochron scepter}.
If you have {Melek, Izzet Paragon} and {Galvanoth} out at the same time,
you get the top card of your library cast and copied for free before you draw.
Add {Pyromancer Ascension} and you'll get three instances!
And throw in a {Nivmagus Elemental}, you can have him grow by exiling unwanted copies or dead spells!
{Reverberate} a {Increasing Vengeance} cast from a graveyard. Make the copy copy the copy, making infinite copies.
This works because Increasing Vengeance from the graceyard makes TWO copies per resolution, instead of Reverberate's only one.
Now have {Nivmagus Elemental} eat off the infinite copy loop, becoming an infinitely large creature.
