After a cipher is encoded on a creature, if said creature dies, what happens to the cipher card?  Does it stay exiled or can it then be placed on another creature card?

The spell that you exiled was only ciphered onto that one creature. Once it dies, the spell will still be exiled, but you cannot interract with it, or cipher it onto another creature.

What if said creature with a cipher spell linked to it returns from the graveyard will it have that cipherd spell

No. Once the creature leaves the battlefield, the encoded effect no longer applies.
