I'm not quite sure how Extort works yet....
I practically pay one extra Mana when I cast a spell with extort. But if a creature or enchantment with extort is already on the battlefield can I pay the extra mana on some other spell even if it doesn't have extort and have it deal one damage?

Its like this. You cast, lets say, a creature with Extort. You dont pay anything now. However the next time you cast a spell you may pay an extra {B/W} for each permanent with Extort you control. So lets say you have 2 extort creatures on the battlefield. You cast {Ultimate price}.
You can then pay 2 more mana and you'll gain 2 life for each opponent and they'll lose 2 life each.

If you control a permanent that has exort, then whenever you cast a spell (before it can resolve or be responded to), you may pay black or white, and gain one life and have an opponent lose one life.

It doesn't matter if the spell you cast does not have exort; as long as you control a permanent with exort on the battlefield, you may pay 1 every time you cast any spell, and do the effect.

Exort stacks. For instance, if you control two permanents with exort, whenever you cast a spell you may pay 1 to exort, then you can do it again. But you must pay 1 for every new exort. For instance, if you have 3 permanents with exort, you can't pay 1 and gain life and have each opponent lose 3. You would have to pay 3 mana.

Edit: by the way, it's each opponent. At the start of this post I said you can have an opponent lose life. I am tired, I forgot that it effects each opponent, and does not target. Small detail though :p
