Would {Thragtusk} + undying yield two tokens or does undying not work like that?
Okay so the first time thrag dies you get a token. End of turn he returns you gain another 5, next turn he dies again you get another token but stays in the yard unless you {cloudshift} him or something
Undying doesnt wait till end of turn.
Thragtusk dies with undying, his leaves triggers and makes a beast.
His undying triggers and returns him to the battlefield and you gain 5 life.
He is now a 6/4 thragtusk due to a +1/+1 counter.
The next time he would die a beast would be made and as long as he maintained his counter then he wont come back due to undying.
All of this assuming your opponent has no response to any of the triggers.

Not necessarily "instant"
It is the same as {vexing devil} entering te battlefield or {worldspine wurm} entering the graveyard. There is the action an then the trigger followed by a chance to respond (at instant speed only since the stack is not empty due to a trigger being on the stack) and finally the resolution where the creature is returned to the battlefield.
Could {cloudshift} really bring something back from the graveyard?

No what it does do is make kill spells fizzle if you play it right
E.g. I target your {Thragtusk} with {murder}. In response, you {cloudshift} thragtusk, making him a new object. My murder fizzles because the thragtusk it targeted is nowhere to be found. AND you get a 3/3 token and 5 life.

Just to clarify but if you decide to give {Vexing Devil} {Undying} you need to cast the undying
before you allow the opponent to choose. Otherwise you can't respond to the opponent's choice.

You can't give it undying while it is still a spell on the stack, which is where your opponent makes the choice.
If it is already on the BF and dies and you give him undying then your opponent will get to chose again from his ETB trigger.
