So if I play {Phantom Centaur} and attach {Banshee's Blade} and attacked with it twice, if all of its counters are gone does that mean it never takes damage or do I have to remove a counter for that to work
Phantom centaur requires you to remove a counter (cost) to prevent the damage to him (effect). If you don't have any counters on him to pay the cost, you can't get the effect.

You're right, my mistake! I should have probably read the card a little closer. There's no cost:effect colon. My apologies, Jroch is correct! Damage is prevented, counters or no counters. Then, if counters are present, one is removed.
