Title: Medicine Runner And +1 counters

If a creature has a +1 counter on it from an active Enchantment like Call to arms, what happens when Medicine Runner enters the battlefield? If the creature is targeted, is the counter removed permanently?

{call to arms}  doesn't give counters. It's an effect. Example. {increasing savagery} adds counters. {crusade} is an effect.

And yes {medicine runner} will remove any counter permanently.
