I am confused with the way this card works. When you put the counters on this card do they continue to stack from the previous rounds?

Previous rounds? Previous turn, yes. +1/+1 counters stay on the creature changes zones

Thank you for the correction. Next question now that I know from turn to turn that the counters stay. Whenever a new creature is put into the battlefield the counters are put on it. Do those counters stack with the previous counter total?

The Goliath will get bigger and bigger with each counter. They don't go away until he leaves the battlefield and you always add up all +1/+1 counters.

Most abilities like this are permanent unless otherwise stated.

For example, alot of abilities like {Bloodthrone Vampire} say "until end of turn". THEN it would not carry over to next turns. But counters are permanent until that creature with counters moves zones or unless an ability says to remove a counter.
