Not in my experience. But I'm not familiar with the rules WotC endorsed with the release of the Commander decks.

Nope. If you cast Phage from the command zone you will lose, unless Platinum Angel is rocking the field next to her. Or your opponent has an abyssal persecutor staring at you. Could also try Lich's Mirror if you're feeling dangerous.

Phage also works with torpor orb and sundial of the infinite.
